The Committee Review Form is generated after the AI-assisted assessment step. It organizes the submitted request and supporting analysis into a review-ready document.
What it includes
Request details, business justification, systems impacted, AI risk level, executive summary, operational impact, technical impact, compliance findings, security findings, contractual findings, risk assessment, recommendations, discussion topics, and historical intelligence.
Why it matters
The form is designed to help reviewers quickly understand the request, the likely impact areas, the open questions, and the implementation concerns that should be considered before a decision is made.
Committee Disposition Record
The Committee Disposition Record is generated after the simulated committee review step. It documents the final simulated outcome of the request.
What it includes
Committee decision, decision rationale, required conditions or next steps, review date, request status, original request details, and the supporting governance analysis used during review.
Why it matters
The disposition record creates a traceable final artifact showing what was decided, why it was decided, and what conditions or follow-up actions were identified.
